Pharmacia & Upjohn has recalled the following batches of Tamofen (tamoxifen) tablets 20mg: 806144, 806145, 818291, 818537, 823851, 823855, 823866, 952945, 954762 and 956674. All are packs of 30 tablets, distributed between November 16, 1998, and September 15, 1999. The recall follows discovery of two typographical errors in the text of the patient information leaflet supplied with these batches. In the section "How should you take your medicine" the leaflet states "For breast cancer the usual dose is one tablet twice daily," and "For anovulatory infertility the number of tablets can vary (1 - 4 twice daily)." For both indications, the word "twice" is incorrect and should not appear. Recipients of these batches should contact Pharmacia & Upjohn at Milton Keynes (telephone Lorna Allen, customer services, 01908 603888) to arrange return and replacement.
