The Council of Europe has agreed a minimum data set that should be included in all prescriptions for prescription-only medicines.
The council's committee of ministers resolved on March 15 that prescriptions should include the prescriber's name, address and occupation, a hand-written signature, the patient's name and personal details, along with the name, dosage and pharmaceutical form of the medicine and instructions for treatment.
