Medical records

Guidelines

From Ms J. S. Nicholls, MRPharmS, and Mr D. Webb, MRPharmS

Further to the “Broad Spectrum” article “Pharmacy and professionalism in health care” (PJ, September 30, 2000, p485), which discusses the need for professional accountability and documentation, we have recently adopted a set of regional guidelines for writing in patients’ medical records. The guidelines are designed to assist clinical pharmacy managers to establish locally agreed policies and to support the necessary local training of clinical pharmacists. The guidelines are drawn from a range of experience, both in the literature and in practice. A copy is available on request (e-mail clinical@ntpharm.co.uk).
