Pharmacies in west Wales to record one week's near misses

During the first week in March, pharmacies in Pembrokeshire, Wales, will be recording any near miss-errors made during dispensing.

Chris Martin, chairman of the Pembrokeshire Pharmacy Forum, said that this will enable pharmacists to identify why such mistakes happen. The results are expected to be fed back to the local health group. A similar scheme for prescribing errors is also planned.

An audit of clinical incidence recording is also under way in the area. Pharmacists will document within their own pharmacies mistakes made during dispensing, where the medicines have left the pharmacy and an error has been reported.

Mr Martin added that the forum has also received £2,250 from the LHG. This will allow 30 pharmacists to employ half-day locums so that LHG representatives can meet them to discuss development plans.
