Work together, pharmacists told

Continuity of pharmaceutical care can be improved by hospital and community pharmacists working together, said Professor GEORGES ZELGER, Nord Vaudois and de la Broye hospital, Switzerland. This can only happen if pharmacists get to know each other better, for example through joint education meetings. He added that if clinical pharmacy is to gain credibility, communication between pharmacists is essential.

In Switzerland, early results from a study, in which patients were discharged from hospital with a document that included a prescription and care plan, had demonstrated health professionals in the community had sufficient information to care for the patient effectively.
