Prescription Products

Tamiflu capsules

Composition: Oseltamivir (as phosphate) 75mg.

Presentation: Capsules.

Class: Antiviral.

Indications: Treatment of influenza in adults and children one year of age or older who present with symptoms typical of influenza, when influenza is circulating in the community. Post-exposure prevention in adults and adolescents aged 13 years or older following contact with a clinically diagnosed influenza case when influenza is circulating in the community.

Dosage: Treatment, 75mg twice daily for five days. Treatment should be initiated as soon as possible within the first two days of the onset of symptoms. See SPC for dosage in children. Post-exposure prevention, 75mg once daily for at least seven days. Prevention during an influenza epidemic in the community, 75mg daily for up to six weeks.

Major cautions: Safety and efficacy of oseltamavir in immunocompromised patients have not been established. Dose adjustment is recommended in adults with severe renal insufficiency. Care should be taken when prescribing oseltamivir in subjects when taking co-excreted agents with a narrow therapeutic margin (eg, chlorpropamide, metho-trexate, phenylbutazone. Oseltamivir should not be used during pregnancy unless potential benefit to the mother justifies the potential risk to the foetus. It should be used during lactation only if the benefit for the mother justifies the potential risk to the infant.

Single dose vials of Vaqta Paediatric (formaldehyde-inactivated hepatitis A virus) vaccine have been launched (Aventis Pasteur MSD); net price, 0.5ml £15.65. All batches of Vaqta Paediatric vaccine were recalled last year (PJ, 8 December 2001, p812).

SPC changes

Neoral and Sandimmum

The summaries of product characteristics for Neoral (ciclosporin) capsules and oral solution and Sandimmum (ciclosporin) concentrate for infusion have been updated (Novartis Pharmaceuticals). The special warnings and precautions for use sections now state that ciclo-sporin may increase the risk of benign intracranial hypertension. If benign intracranial hypertension is diagnosed, ciclosporin should be withdrawn. Optic disc oedema, including papilloedema secondary to benign intracranial hypertension, is included as a very rare undesirable effect. Non-cardiogenic pulmonary oedema has been added to the undesirable effects section of the Sandimmum concentrate for infusion SPC. See SPC.
