Pharmacy projects win HSJ awards
Two projects involving pharmacists were winners of this year's Health Service Journal awards.
The HSJ patient safety award went to the integrated medicines
management project (PDF 75K)
at the United Hospitals Trust in Northern Ireland (PJ, August
16, p214). The project has shown that patients who take the right dose
of the right medicine at the correct time show better improvement in
their conditions and have shorter hospital stays and fewer readmissions.
It involves establishing complete drug records for patients when they
are admitted and discussing medication with patients during their stay.
On discharge, all relevant medicines information is then faxed to the
patient’s general practitioner and community pharmacist.
The HSJ chronic disease management award was won by South East Sheffield
Primary Care Trust for the identification of previously undiagnosed or
poorly managed coronary heart disease and diabetes. More than half of
the 213 patients seen at a community pharmacy by a practice nurse needed
further investigation. |
