Prescription forms should be standardised

Further to Bob Dunkley’s letter (PJ, 31 January, p119), I am also concerned about the confusing variation in format of the blue FP10(MDA) prescription forms. Safe, correct and prompt dispensing of these would be facilitated if they had a standard layout, perhaps with a grid asking for the dose to be given on a particular day. The maximum supply on FP10(MDA) is 14 days, therefore it should be simple to devise such a form. Currently, we have buprenorphine prescriptions, some of which give a daily dose in milligrams while others give it in number of tablets. There should be standardisation so that the dosage is patently clear.
