New medicines

Subgam
Composition: Human normal immunoglobulin. Each millilitre contains 140–180mg human protein of which at least 95 per cent is immunoglobulin G.
Presentation: Solution for subcutaneous infusion or intramuscular injection, depending on the indication.
Class: Immune sera and immunoglobulins.
Indications: Replacement therapy in adults and children in primary immunodeficiency syndromes. Replacement therapy in myeloma or chronic lymphatic leukaemia with severe secondary hypogammaglobulinaemia and recurrent infections.
Dosage: Dependent on the indication and may need to be individualised for each patient depending on pharmacokinetic and clinical response. A loading dose of 0.2–0.5g/kg may be required. Maintenance doses are administered at repeated intervals to reach a cumulative monthly dose of 0.4–0.8g/kg. See SPC.
Precautions: Patients could develop shock if Subgam is accidentally administered into a blood vessel. The recommended infusion rate stated in the SPC should be adhered to. Patients should be closely monitored and carefully observed for any adverse events throughout the infusion period. See SPC.
Contraindications: Must not be given intravenously. Must not be administered intramuscularly in cases of severe thrombocytopenia and in other disorders of haemostasis.
Side effects: Adverse reactions such as chills, headache, fever, vomiting, allergic reactions, nausea, arthralgia, low blood pressure and moderate low back pain may occur occasionally.

SPC changes

Azactam
The undesirable effects section of the summary of product characteristics for Azactam (aztreonam) now includes hypersensitivity (anaphylaxis, angiodema and bronchospasm) and gastrointestinal bleeding as effects that have been reported with the drug (Bristol-Myers Squibb). Seizures and dyspnoea have been included as rare events. See SPC.

Uftoral
The summary of product characteristics for Uftoral (tegafur and uracil) capsules now states that increased phenytoin plasma concentrations resulting in symptoms of phenytoin intoxication have been reported with the concomitant use of Uftoral and phenytoin (Bristol-Myers Squibb). Patients taking phenytoin concomitantly with Uftoral should be regularly monitored for increased phenytoin plasma concentrations. See SPC.

Discontinued products

Cefuroxime
Cefuroxime injection 750mg and 1.5g vials will be discontinued after 1 September (Lilly).

Co-Betaloc SA
Co-Betaloc SA (metoprolol tartrate and hydrochlorothiazide) tablets are being discontinued, expectedly on 1 August (Pfizer).

Efexor tablets
Efexor (venlafaxine) immediate release tablets 50mg are being discontinued (Wyeth). The company expects this to happen at the end of August. All other presentations of Efexor immediate release tablets and Efexor XL modified release tablets remain available.

Zorac
The 60g pack of Zorac (tazarotene) gel 0.05 per cent and 0.1 per cent has been discontinued (Allergan). The 30g pack remains available.

Products miscellany

Acnisal, Meted, Occlusal and Pentrax
From 2 August the distribution of Acnisal (salicylic acid) topical solution, Meted (salicylic acid and sulphur) shampoo, Occlusal (salicylic acid) application and Pentrax (coal tar) shampoo will be transferred from DermaPharm to Alliance Pharmaceuticals (customer services tel 01249 466974, fax 01249 466976, medical information tel 01249 466966, fax 01249 466977). The marketing authorisations for these products will transfer at a later date. The products will remain in DermaPharm livery until Alliance packaging is available.
