Resuscitation errors common

Medication errors are common at all stages of paediatric resuscitation, say researchers.

They analysed eight mock resuscitations and found that out of 125 orders for medicines, nine dosing errors occurred, five of which were intercepted before administration. Four ten-fold dosing errors were identified. Nine out of 58 syringes analysed contained a deviation of 20 per cent or more from the ordered dose (BMJ 2004;329:1321).
