MI pharmacists should report errors via new database

A new database of medication errors and near misses for use by those working in medicines information has been set up by the UK Medicines Information clinical governance working group.

The scheme will act as an internal monitoring service within NHS medicines information services and is intended to complement, rather than replace, existing reporting systems within the NHS.

Users will therefore need to report any medication errors and near misses through their primary care trust’s system as well as into the new medicines information database website.

An additional database is needed, Julia Horwood of the UKMi clinical governance working group explained, because, although PCTs should record all medication errors, the data cannot be easily extracted and do not contain sufficient detail to allow for the analyses that UKMi need to conduct.

The data gathered by the new site will, in time, be forwarded to the National Patient Safety Agency.
