The NHS Information Standards Board has launched a set of standards for documenting patient information (PDF 100K).

The standards are based on best practice from the Nursing and Midwifery Council, General Medical Council and Health Professions Council, and aim to provide a single model for recording patient records throughout the NHS in England.
