Having a pharmacist on post-admission ward rounds improves the accuracy of drug history taking, reduces prescribing costs and decreases potential risks to patients from medication errors, a study conducted at Northwick Park Hospital, Harrow, has shown (Quality and Safety in Health Care 2005;14:207).
