Pharmacists in Wales to use different MUR forms

Pharmacists in Wales will not be using the same medicines use review form as those in England, Community Pharmacy Wales announced this week.

Three claim forms associated with MURs are available for pharmacists in Wales: one for MURs carried out, one for pharmacists to register their competency to carry out MURs and one for self-certified registration of premises.

Details of pharmacists registered to carry out MURs will be entered onto an all-Wales database, to which all local health boards will have access.

The forms can be downloaded here

Correction
Pharmacists in England and Wales will be using the same medicines use review forms to record the MUR process and not different forms, as suggested. However, pharmacists in Wales will use two different forms to register themselves and their premises to undertake MURs and another form to claim reimbursement for MURs undertaken. These three forms are used in Wales instead of the Prem1 form used in England.
