Community heart failure clinic awarded £5k

Organisers of a community heart failure service in Glasgow have been awarded £5,000 to evaluate its impact on patients. The service won the Guild of Healthcare Pharmacists/National Pharmacy Association/Merck Sharpe & Dohme joint pharmacy award.

Almost 400 patients have so far been referred to a community pharmacist for advice on heart failure medication and for monitoring. Patients are referred by prescribing support pharmacists and hospital pharmacists or heart failure liaison nurses.
