New refund form for Scotland

A new form for receipt and refund of NHS prescription charges is being introduced in Scotland. The new HCS(R) form is the Scottish equivalent of the English FP57 form.

Community pharmacists will receive a supply of the new forms via NHS boards and, on their receipt, should destroy existing stocks of FP57 forms. All other arrangements for refunding prescription charges remain in place: patients should still apply for refunds by post.
