Patients most at risk of unplanned or unnecessary hospital admissions may be more accurately identified by a new NHS model.

The Combined Predictive Model uses accident and emergency, inpatient, outpatient and GP data to identify patients who may benefit from early intervention. It can also categorise people according to their risk of hospital admission.
