Guidance issued in Scotland on private CD requisition forms

Guidance (PDF, 170K) on the use of private requisition forms for Controlled Drugs was issued this week in Scotland.

It explains that from 1 January 2008, all forms (the original form, not a copy) for private requisitions of schedule 1, 2 and 3 CDs must be forwarded to NHS National Services Scotland after dispensing. Pharmacists must either stamp or indelibly write on the requisition form the name and address of the pharmacy supplying the CD.

In addition, a standard requisition form (CDRF) has been produced for private requisitions of schedule 1, 2 and 3 CDs. An example of the form is published within the guidance. However, the form’s use is not compulsory provided all the legal requirements are met. Current NHS stock order forms (GP10A) in Scotland meet the new requirements so can continue to be used for NHS requisitions of CDs.

The guidance also adds that, although it will not be a legal requirement, it is good practice for a pharmacy that supplies another pharmacy with a CD to submit a requisition form to NSS for processing.

Similar guidance was recently issued in England (PJ, 27 October, p462) but has not yet been developed for Wales.
