Exercises in clinical checking
Readers may wish to note the following clarifications
and changes to the clinical checking article published in the September
HP (p219–25).
Figure 8 (p221).
For folinic acid, the circles on 14:00 and midnight (MN) should be ignored.
For enoxaparin, the circle should be around 18.00 and not 8.00. Both drugs
were administered as described in the figure.
Figure 9 (p221).
The asterisk on the 10mg doses of midazolam indicates divided dose administration.
Figure 10
(p222). The dates on the chart should be as described here. Vancomycin,
meropenem, metronidazole and the first doses of both atracurium and methylprednisolone,
were prescribed and administered on 10 April. Vitamin K and the second
doses of both atracurium and methylprednisolone were prescribed and administered
on 11 April.
Figure 12
(p223). The dose frequencies of prednisolone and tacrolimus were
interchanged. Prednisolone should be taken once daily at 8:00 (but not
on the days that methylprednisolone is administered) and tacrolimus should
be taken twice daily at 8:00 and 22:00. Azathioprine 50mg once daily at
8:00 should be added to the immunosuppression drugs.
Solution to prescription
2 (p225). For co-trimoxazole, the dose and dose frequency
are correct, therefore there is no need to advise the prescriber. For
meropenem, administered once only (see Figure 10), the dose would only
require adjustment if continued.

may have been caused by the errors.
