Medicines management information sharing in developmentA single assessment tool for patient review that allows health care workers and social services to share the same patient information is being developed by the National Medicines Management Collaborative in north east Derbyshire. Peta Dodsworth, project facilitator for the collaborative, said: "We are developing electronic templates for use in primary care and we are getting to the stage of trying to work out how we are going to share the information with social services." Social services currently collect information on medicines use and this is the sort of information needed for medication reviews. Ms Dodsworth said: "At the moment, social services don't have the capability to do it electronically, so we are trying to figure out a way for them to do it with paper, and practices to do it paperless."
