Essential elements to be included in clinical management plans
Details of the clinical management plan that the Government describes as the corner stone of supplementary prescribing have just been revealed.
Two recommended CMP templates, which are linked to whether the supplementary
prescriber has full access or only partial access to the patient record,
have been drawn up and are available on the department of health website.
The CMP has to be agreed between the supplementary prescriber — either
a pharmacist or a nurse — and the independent prescriber and the
patient. All three parties are expected to sign the plan.
If pharmacists do not have access to the full patient record the CMP
must include details of current medication, a short medical history and
contact details of the independent and supplementary prescribers.
Other details required, which are common to both CMP templates, include
a list of medicines which the supplementary prescriber can prescribe,
any relevant clinical guidelines or protocols, how often the medicines
should be reviewed and who is responsible for monitoring the patient.
Department of Health regulations also require that the CMP includes details
about any limits to the medicine dose or strength, and the circumstances
in which the supplementary prescriber should seek advice from the independent
prescriber.
The CMP templates have been published to help those pharmacists who are
thinking of becoming supplementary prescribers. The Government intends
to have 1,000 pharmacist supplementary prescribers by next year.
