This article looks at the management of chronic diseases by the multidisciplinary team in primary care and also highlights the role for clinical pharmacists The author concludes that quality of care can be improved if management is by an appropriately trained multidisciplinary team with designated roles, and provided doctors are willing to share care effectively in a practice which is sufficiently organised to support the changes.
