Medication Review in Residential and Nursing Homes

Madam, - I am a nursing home pharmacist inspector currently undertaking a critical review entitled "Medication optimisation/ review in long stay care homes for the elderly - project or reality?" I intend to investigate the extent to which medication review for this group of patients is the subject of feasibility studies and projects or continuous review, which groups of pharmacists are, or should be, involved in. Also to investigate the barriers to change, including quality issues and costs.
I would therefore like to hear from pharmacists in any part of the profession, who are, or will be involved with medication review for residential and nursing homes. I am particularly keen to hear from pharmacists conducting regular reviews within the same homes and from those who have completed successful projects but have been unable to implement regular reviews.
