The standardised ginseng extract described in a news item (PJ, 29 October, p541) was made up of 80 per cent poly-furanosyl-saccharides and 10 per cent protein formulated from the roots of North American ginseng. The capsules contained 200mg of freeze-dried extract.

The Guildford and Waverley Primary Care Trust team which won the NHS innovations award (PJ, 22 October, p506) also included Louise Robinson and Tina McKee, who were not pictured.

CORRECTION Pharmacy diabetes Smartgroup
The email address to join the Diabetes UK Smartgroup (PJ, 8 October, p435) is pharmacydiabetesnetwork-subscribe@smartgroups.com. “Smartgroups” is not hyphenated.

CLARIFICATION Healthy returns
Payments presented at the UniChem convention for services (PJ, 8 October, p448 PDF (90K)) were incorrect. The correct fees are: warfarin clinic, £85 per patient per year; hip fracture review clinic, £20 per consultation; and stroke clinic, £500 set up fee plus £300 for two days each month.

Broad spectrum
The Table in the Broad spectrum article “Thinking the unthinkable — pushing the boundaries of pharmacy skill mix”, which appeared in the 10 September issue (p306), should have been titled “Task/competency matrix for applying skill mix”.

Sinthrome
The SPC change for Sinthrome (acenocoumarol) tablets (PJ, 20 August, p223) was incorrect. Alliance now says that cranberry juice should be avoided in patients taking Sinthrome due to a theoretical risk of enhanced anti-coagulation, not coagulation.

Retention fees
Retention fees for non-practising pharmacists are to rise by 30 per cent from £46 to £60, and not 25 per cent as previously stated (PJ, 13 August, p199).

Stanley Blum’s letter
Stanley Blum mentioned Staphylococcus typhimurium in his letter (PJ, 30 July, p139). It should have read Salmonella typhimurium.

Howard McNulty’s letter
Howard McNulty’s letter (PJ, 6 August, pp162–3) was published without several lines due to a production error. The first column of p163 should have started

“national boards looking after professional interests there will need to be clearer demarcation and funding of these two separate roles and for the boards. It will be important to develop separate professional and regulatory budget elements to ensure both are able to break even. Publications should be used to subsidise professional elements and not regulatory ones. Other professional bodies are better at looking after students and”.

MUR forms
Pharmacists in England and Wales will be using the same medicines use review forms to record the MUR process and not different forms for this process as suggested in a recent news item (PJ, 16 July, p72). However, pharmacists in Wales will use two different forms to register themselves and their premises to undertake MURs and another form to claim reimbursement for MURs undertaken. These three forms are used in Wales instead of the Prem1 form used in England.

Bill Brookes has asked us to point out an error in his letter (PJ, 16 July, p84). What he intended to write under point 2(a) was: “Charge a realistic but not exorbitant rate to those seeking rest and recuperation.”

Community collaborative
In last week’s report of concern that pharmacies were not being reimbursed costs incurred in taking part in the Department of Health’s community pharmacy framework collaborative (PJ, 16 July, p75), the National Pharmacy Association claimed the scheme had £4m of DoH funding. The correct figure is £2.1m.

Gideon Kotey
In our recent report on the Pharmaceutical Care Awards for 2004 (PDF 730K), the name of Gideon Kotey, was misspelt in a photograph caption (PJ, 2 July, p16).

Reshma Gandecha
We have been asked to point out that Reshma Gandecha is a health informatics researcher and a pharmacist and that the views expressed in her article “What the national IT programme means for pharmacy and pharmacists” (PJ, 9 July, p56 PDF (260K)) do not necessarily reflect those of Essex Strategic Health Authority or the Essex NPfIT programme, although her NPfIT research covers the NPfIT trajectory in Essex and Cambridgeshire.

Lithium clinic
In the report of the lithium clinic that won a Pharmaceutical Care Award (PJ, 2 July, p16 PDF (730K)) the third sentence in the fourth paragraph should have read: “92 per cent have had their lithium levels checked within the previous three months and 99 per cent their serum creatinine and thyroid stimulating hormone levels checked within the previous six months.”

Radioactivity dose
The dose of radioactivity above which advisory committee approval is required 0.1mSv, not 1mSv as stated in the article on microdoses in last week’s Journal (PJ, 9 July, p54).

CLARIFICATION: Rajiv Sarna
Our report of the prosecution of Rajiv Sarna for making a fraudulent Essential Small Pharmacy Scheme claim (PJ, 4 June, p668), was based on information provided by NHS Counter Fraud Services. NHS Counter Fraud Services now accepts that Mr Sarna would have been entitled to £15,600 of the £23,000 he claimed. Mr Sarna returned £23,000 to the NHS before his trial, not before sentencing as reported, and will now receive a refund of £15,600.

CORRECTION: New member fee
The Royal Pharmaceutical Society’s new member registration fee is £125, not £110 as stated in last week’s issue (PJ, 2 July, p10).

Palonosetron (Aloxi) is licensed for the prevention of nausea and vomiting associated with moderately emetogenic cancer chemotherapy, not just acute nausea and vomiting, as stated last week (PJ, 11 June, p701).

Etanercept
Etanercept is licensed for the treatment of adults with moderate to severe plaque psoriasis and not as stated in our CPD article (PDF 310K) last week (4 June, pp689–92). The recommended dose for this indication is 25mg twice weekly. The licence for this indication was granted in November 2004.

Alan Nathan
Alan Nathan, pharmacy writer and consultant, is a visiting lecturer at King’s College London and not as stated last week (PJ, 28 May, p652).

Duac Once Daily gel
The price of Duac Once Daily gel (Stiefel) 50g is £19.90, not £19.95 as The Journal was previously informed (PJ, 7 May, p541).

Hibiscrub
Hibiscrub is a Regent Medical product, not an SSL product as stated previously (PJ, 16 April, p449).

Hycosan eye drops
Hycosan (hyaluronic acid) eye drops 0.1 per cent (Bausch & Lomb UK) are classified as a medical device, not a GSL product as The Journal was previously informed (PJ, 21 May, p609).

BNF CORRECTION: DaunoXome
There is an error in 48th edition of the British National Formulary (September 2004) for the DaunoXome (daunorubicin) entry (section 8.1.2, p427). The side heading reads “lipid formulation for intrathecal use” but should read “lipid formulation”. DaunoXome is for intravenous use, not for intrathecal use. The error will be corrected in BNF 49 (March 2005).

Galanthamine structure
There was a mistake in the chemical structure of galanthamine printed in the 18/25 December 2004 issue of The Journal (p906). The correct structure is printed below.

Code of Ethics: Online pharmacy service
There were two errors in the text supplied to The Journal for the Code of Ethics changes agreed at the December 2004 Council meeting (PJ, 18/25 December 2004, p889). Both relate to the revision of Service Specification 9, concerning online pharmacy services.

In the introductory text, the penultimate sentence should read: “Details of how to confirm the registration status of the pharmacy and pharmacists must be provided.”

In paragraph (c)(i) the second sentence should read: “A record must be kept of the pharmacist assuming professional responsibility for the recommendation and this pharmacist must be identified to the patient.”

Sam Bolanakis’s name was misspelt in our December issue of Prescribing & Medicines Management (PM3).
