Raynaud’s phenomenon
In our continuing professional development article on Raynaud’s phenomenon (PJ, 8 December 2007) the dose (unlicensed) for iloprost intravenous infusion (p650) should have read 2ng/kg/min and not 2ng/kg.

Schering Health advises that the dose is adjusted according to individual tolerability within the range of 0.5 to 2.0ng/kg/min over six hours daily.

Treatment periods of three to five days are often sufficient in Raynaud’s phenomenon to achieve improvement over several weeks.

ESCP report
Dosing information about 5-fluorouracil was incorrect in The Journal’s report of Julie Fisher’s presentation at the European Society of Clinical Pharmacy conference (PJ, 24 November, p597).

5-fluorouracil is administered to patients over seven days, not five days as stated (ibid, p598).

CLARIFICATION: Clarke Inquiry panel
Information provided by the Clarke Inquiry about panel member Peter Owen was incorrect. Mr Owen ceased to be secretary general of the Institute of Chartered Accountants in 2003 (PJ, 3 November 2007, p493).

CLARIFICATION: Digital CD log
Nader Siabi has now said that the Royal Pharmaceutical Society has not approved his software for keeping Controlled Drug records (PJ, 3 November 2007, p490).

CLARIFICATION: Eprex
Eprex (epoetin alfa; Janssen-Cilag) solution for injection 40,000 units continues to be available in prefilled syringes; only the vials have been discontinued (PJ, 3 November, p495).

Nomad monitored dosage system was developed by Surgichem not Protomed as suggested in last week’s issue (p463).

Atriance
The price of Atriance (PJ, 15 September, p286) should have read “6 x 250mg/50ml vials, £1,332”, and not as stated.
The Journal was supplied with incorrect information.

Heather Simmonds
In the letter we published from Heather Simmonds on 22 September (p325), we wrongly attributed the designation “MRPharmS”. Mrs Simmonds is not a member of the Royal Pharmaceutical Society.

The winners of this year’s Servier awards were presented with grants and cheques by Alasdair MacCulloch of Servier Laboratories Ltd and not as stated in a British Pharmaceutical Conference report (PJ, 15 September, p298).

CLARIFICATION: Gerald Wilkinson tribute
The tribute to the late Gerald Louis Wilkinson, which appeared last week (p276), was written by John L. Woodward, MRPharmS.

CORRECTION: Aliskiren
The second paragraph within the Panel “How Aliskiren works” in last week’s News section (PJ, 8 September, p251) should have read: “Aliskiren directly inhibits renin, which converts angiotensinogen to angiotensin-I. Angiotensin-I is then converted to angiotensin-II (ACE inhibitors block this step), triggering vasoconstriction (ARBs stop angiotensin-II receptor binding).”

Information pharmacists
The period on Thursdays when Royal Pharmaceutical Society information pharmacists can be contacted was incorrectly given last week (p271). The correct period is 10am to 5pm, and not 9am to 5.45pm as stated.

Registration examination
There was an error in the list supplied to The Journal of successul candidates in the Royal Pharmaceutical Society’s registration examination published on 28 July (p111). The name Sajeel Abid is incorrect.

The correct name is Sajeel Abrar.

The proposed fees for premises registration, retention and restoration given last week in the Table on p129 were incorrect.

The correct proposed fees are announced this week (p162).

Correction: Industry code breaches
A news report last week (p65) should have said that Bayer, GlaxoSmithKline and Roche were publicly reprimanded in our 7 July issue (inside front cover) and not as reported.

CLARIFICATION: Code of Ethics
The Code of Ethics for Pharmacists and Pharmacy Technicians distributed with last week’s issue is the actual code that comes into effect on 1 August, and not a preview as was suggested (PJ, 21 July, p83).

Registrants are asked to retain it.

CLARIFICATION Journal packaging
In the editor’s response to David Knott’s letter (PJ, 19 May, p586) we gave misleading information provided by the supplier about the cost of distributing The Journal in biodegradable polywrap. We have investigated the options further and it is cost effective to switch to biodegradable polywrap, which will take place from this issue.

CORRECTION Brian Curwain’s letter
Brian Curwain (PJ, 16 June, p704) was described as “chief pharmacist” at Hampshire PCT. This was an editorial error; he does not hold this post and wishes to make this clear in fairness to his colleagues.

Counterfeit lot number
One of the lot numbers of counterfeit Zyprexa was listed incorrectly as A21645 in last week’s product recall (PJ, 2 June, p641). The number should have read: A216454.

NAWP president
Hazel Baker, MRPharmS, has been appointed president of the National Association of Women Pharmacists, not Veronica Pearson as reported (PJ, 5 May, p519). The Journal was supplied with erroneous information.

CLARIFICATION: NPA board
Further to last week’s correction (PJ, 5 May, p519) to the inaccurate list of National Pharmacy Association board members provided by the NPA, the NPA board member for North West London is Hiten Patel.

Carole Brown’s letter
The second sentence of the third paragraph of Carole Brown’s letter (PJ, 5 May, p524) should have read: “On returning to the surgery, pharmacists discussed the reviews with the GP and suggestions were actioned.”

The list of National Pharmacy Association management board members (as supplied by the NPA) printed last week was incorrect (PJ, 28 April, p476). As stated, Dilip Joshi is chairman of the board for 2007–08. CLARIFICATION: Queen’s Awards
The Queens Awards for Enterprise reported in the news pages of last week’s Journal (PJ, 21 April, p448) are separate awards. The University of Nottingham School of Pharmacy received an award for “Pharmaceutical applications of nanotechnology” and Molecular Profiles Ltd, for “Contract analysis and research services to pharmaceutical and biomedical companies”.

Erasure from Register
The names of 34 persons who had indicated their wish to retire voluntarily from the Register of Pharmaceutical Chemists were included erroneously in the Royal Pharmaceutical Society’s official notice listing pharmacists erased from the Register for non-payment of the retention fee for 2007 (PJ, 7 April, p409).

The Society apologises for the error and for any embarrassment caused.

IPG Committee election
An Official Notice inviting nominations for election to the Industrial Pharmacists Group Committee, published in each issue of The Journal in February, wrongly stated that each nomination had to be signed by at least five group members. In fact, no more than one nominator is required. A corrected notice appears on p293.

McKesson Corporation
It is the US McKesson Corporation that provides services to community pharmacy businesses and not as stated last week (p235).

Society’s assets
The Society’s assets were valued at £5.246m in its 2005 accounts, not £5,246m, as stated in a news item last week (PJ, 24 February, p207).
