Statutory Committee inquiries

The Statutory Committee will meet at the Royal Pharmaceutical Society of Great Britain, 1 Lambeth High Street, London SE1, at 9:30am on Wednesday 20 and Thursday 21 April 2005 to hear the following inquiries:

Wednesday 20 April 2005
1. The Committee will resume its Inquiry into a complaint by the Council of the Society against Mrs Shital Popat (Registration No 84548) and PIL Limited (Id No 1086946) which alleges that various failings in relation to the presence of patientreturned medicines or medicines that have been left uncollected for a significant period of time on shelves in MDS Room, presence of medicines not properly labelled with batch numbers and/or expiry dates, presence of Temazepam in MDS Room, unsatisfactory working conditions, and failure to have in place an adequate system of disposal of patient-returned medicines, may amount to misconduct.

2. An Inquiry into a complaint by the Council of the Society against Mr Robert Nicholson (Registration No 70435) and Mr Pak Kao Ho (Registration No 74861) which alleges that dispensing out of date Premique Cycle tablets, which were unlabelled and unaccompanied by a patient information leaflet, in circumstances where Mr Nicholson and Mr Ho had received repeated advice and a warning from the Society regarding the requirement to check for and remove date-expired stock from the dispensary and Mr Nicholson had received a warning from the Society regarding the dispensing of unlabelled medication, may amount to misconduct.

3. An Inquiry into a complaint by the Council of the Society against Mr Joseph Karl Francois (Registration No 61723) and DH Roberts (Kentish Town) Limited (ID No 1001471) which alleges that the dispensing of 28 x Thyroxine 50mcg tablets labelled “28 Thyroxine tabs 25mcg” with the direction “take six daily” instead of 336 x Thyroxine 25mcg tablets with the direction “take six daily” which was prescribed, particularly in light of the acceptance in 2002 of recommendations from the Society for minimising the risk of dispensing errors, together with the failure to take appropriate steps when the error was discovered, may amount to misconduct.

4. An Inquiry into a complaint by the Council of the Society against Mrs Gillian Eldridge (Registration No 68186) which includes allegations of presenting herself at work on a number of occasions when her breath smelled of alcohol, failing on a number of occasions to arrive on time for work as pharmacist, together with a conviction of driving while unfit to drive through alcohol, and the intention of going to work as pharmacist on same day as road traffic offence whilst unfit through alcohol to undertake such work, may amount to misconduct.

5. An Inquiry into a complaint by the Council of the Society against Mr Nigel Kevin Carpenter (Registration No 79595) which alleges that the operation of a repeat medication service which failed to comply with the service specifications set out in the Society’s Code of Ethics and Standards, the failure to maintain Patient Medication Records in accordance with the Society’s Code of Ethics and Standards, and submitting to the PPA inaccurately endorsed prescriptions, may amount to misconduct.
