Statutory Committee inquiries
The Statutory Committee will meet at the Royal Pharmaceutical Society of Great
Britain, 1 Lambeth High Street, London SE1, at 9.30am on Monday 20 June, Tuesday
21 June , Wednesday 22 june and Thursday 23 June 2005 to hear the following
inquiries:
Monday 20 June
1. An inquiry into a complaint by the Council of the Society against Sejule
Shah (registration number 1069693) which alleges that possession of a quantity
of cannabis together with convictions for driving while unfit through alcohol
and possession of diazepam tablets may amount to misconduct.
2. An inquiry into a complaint by the Council of the Society against Alan
Roch (registration number 67574) and Honeysgreen Pharmacy
Ltd (identification number
1088653) which alleges that failure to have in place professional indemnity
insurance cover in respect of the professional activities carried on in the
pharmacy, failure to ensure that adequate procedures were in place or instructions
given to non-pharmacist staff in relation to the non-arrival of a pharmacist
for duty and the failure between October 2001 and August 2003 to take adequate
steps regarding the supervision of non-GSL medicines, after advice was given
on the subject in October 2001, may amount to misconduct. In addition, a conviction
against Mr Roch for the supply of bendrofluazide, fluconazole and Cilest tablets
when such supply was not made by a pharmacist or by a person acting under the
supervision of a pharmacist.
Tuesday 21 June
3. An inquiry into a complaint by the Council of the Society against Peter
Herman (registration number 63167) which alleges that the commission of offences
of distributing by way of wholesale dealing products otherwise than in accordance
with a wholesale dealer’s license may amount to misconduct.
4. The committee will continue consideration of an application for restoration
by Rajendra Gulubchand Shah (former registration number 76470), whose name
has been removed from the register.
Wednesday 22 June
5. An inquiry into a complaint by the Council of the Society against Rajesh
Chaganbhai Mistry (registration number 91540) and The House
of Mistry Ltd (identification number 1084647) which alleges misconduct relating to obtaining
payment from the Prescription Pricing Authority in respect of repeat prescriptions
that had not been requested nor required by the patient and which were not
dispensed and prescriptions wrongly endorsed with small pack sizes when larger
sizes had been dispensed.
Thursday 23 June
6. An inquiry into a complaint by the Council of the Society against Jayantilal
Raichand Shah (registration number 69625) which alleges that the unlawful
supply of a prescription-only medicine in the absence of a prescription by
a member of staff while he was the pharmacist in charge may amount to misconduct.
