Statutory Committee inquiries
The Statutory Committee will meet at the Royal Pharmaceutical Society of Great
Britain, 1 Lambeth High Street, London SE1, at 9.30am on Monday to Thursday,
21 to 24 November 2005 to hear the following inquiries:
Monday 21 November
1. The Committee will resume its inquiry into a complaint by the Council of
the Society against Peter Chiu (registration number 78926),
Ajay Walia (registration
number 76620) and Anchor Health Ltd (identification number 1086763) which
alleges that the supply of prescription-only medicines without the authority
of a prescription in circumstances where the conditions for a lawful supply
were not satisfied, and the failure to ensure that procedures designed to
minimise risks were formulated and applied, may amount to misconduct.
2. An inquiry into a complaint by the Council of the Society against Maria
Mariscal Navarro (registration number 1078456) and Andrew
Murdock (registration
number 73499) which alleges that a labelling error and failure to deal appropriately
with a complaint concerning the error, may amount to misconduct.
Tuesday 22 November
3. An inquiry into a complaint by the Council of the Society against Keshavlal
Raghavji Jethwa (registration number 63534) which alleges that the persistent
failure to dispose of pharmaceutical waste, and a failure to co-operate with
the Society inspector’s investigation, may amount to misconduct.
4. An inquiry into Oluseyi Michael Alakiu (registration number 808322)
who has been convicted of two counts of the supply of medicinal products
otherwise
than in accordance with a prescription given by an appropriate practitioner,
contrary to Sections 58 and 67 of the Medicines Act 1968; and being knowingly
concerned in the fraudulent evasion of prohibition or restriction on importation
of a class A Controlled Drug contrary to Section 170(2)(b) of the Customs
and Excise Management Act 1979.
Wednesday 23 November
5. An inquiry into a complaint by the Council of the Society against Manherlal
Keshavlal Shah (registration number 63471) and MAM International
Ltd (identification
number 1001688) which alleges that the supply of out-of-date medicines; the
supply of enalapril tablets on which there was no expiry date or batch number;
failure to implement adequate systems for the removal of out-of-date medicines;
the presence in the dispensary of medicines where the date of expiry and
batch number were not present and/or had been removed; the presence in the
dispensary of medicines labelled years previously from other pharmacies for
various patients which were stored among the other, alphabetically arranged,
current stock, indicating that they might be reused; and, the presence of
a number of European products which did not bear a produce licence number,
may amount to misconduct.
Thursday 24 November
6. An inquiry into a complaint by the Council of the Society against Maysa
Jibreel Al-Natsheh (registration number 90768) and Preston
Park Chemists Ltd (identification number 1087052) which alleges that a failure to ensure
that the pharmacist in charge had the requisite knowledge, skills and fitness
to perform the work delegated to him; failure to ensure that he was sufficiently
competent in English; failure to ensure that he was properly informed of
the professional duties he was expected to undertake; failure to supervise
and support the pharmacist in charge; and a failure to exercise proper control
over the business, may amount to misconduct.
