Statutory Committee inquiries

There follows the outcome of recent inquiries heard before the Statutory Committee of the Royal Pharmaceutical Society of Great Britain on Monday 25 September, Tuesday 26 September, Wednesday 27 September and Thursday 28 September 2006:

1. Following the inquiry into a complaint by the Council of the Society against Omotayo Enitan Adekaiyaoja (registration number 1062304), Martyn James Hardy (registration number 71137) and Sainsbury’s Supermarkets Ltd (Identification number 1003528), the committee resolved to take no further action against Mr Hardy or Sainsbury’s Supermarkets Ltd. In relation to Mr Adekaiyaoja, the committee resolved to direct the removal of his name from the Register. The committee found proved the Council’s allegation that the supply of methadone mixture otherwise than in accordance with the direction on a prescription, a number of erroneous supplies by Mr Adekaiyaoja or while he was the pharmacist in charge, an erroneous supply of porcine isophane insulin 30/70 to a patient and a failure to deal with the error appropriately, and a poor understanding of diabetes and its treatment, amounted to misconduct. Under Section 11 of the Pharmacy Act 1954, the direction to remove Mr Adekaiyaoja’s name from the Register is not to take effect until the expiration of a period of three months from the date on which notice of removal was given or in a case where an appeal has been brought against the direction, until the appeal is determined or withdrawn.

2. The inquiry into a complaint by the Council of the Society against Korush Jalali Farahani (registration number 1067185) was adjourned to resume at a later date.

3. The committee adjourned the inquiry into a complaint by the Council of the Society against Samuel Edwin Ashby (registration number R6879) to deliver its determination at a later date.
