Statutory Committee inquiries

The Statutory Committee will meet at the Royal Pharmaceutical Society of Great Britain, 1 Lambeth High Street, London SE1, at 9.30am on Tuesday 25th, Wednesday 26th and Thursday 27th September 2007 to hear the following Inquiries:

1. The committee will hear the new inquiry into a complaint by the Council of the Royal Pharmaceutical Society against Fatehali Abdalhussein Hassanali (registration number 67479) which alleges that the supply of 210ml of methadone 1mg/1ml sugar free mixture against a prescription calling for 120ml; a failure to take adequate steps to confirm the identity of the patient before making the above supply; recording a supply of 120ml in (i) the Controlled Drugs register for methadone supplied, (ii) the patient’s instalment prescription and (iii) the patient’s medication record, when Mr Hassanali knew that he had supplied 210ml; and following the above erroneous supply, a failure to (i) take any or adequate steps to enquire about the patient’s health and/or well being, (ii) take any or adequate steps to recover the excess supply given to the patient or (iii) inform pharmacy staff or his employer of his conduct as set out above, may amount to misconduct.

2. The committee will hear the new inquiry into a complaint by the Council of the Royal Pharmaceutical Society against Shiv Kumar Sharma (registration number 90720) which alleges that conduct contrary to: Part 2 A.1(a) of the Code of Ethics and Standards, in that he did not have the requisite fitness for the tasks to be performed; Key Responsibility 1 of the Code of Ethics and Standards, in that he did not at all times act in the interests of patients and other members of the public; Key Responsibility 2 of the Code of Ethics and Standards, in that he failed to ensure his performance was of a high quality; and Key Responsibility 3 of the Code of Ethics and Standards, in that he failed to adhere to accepted standards of personal and professional conduct and engaged in behaviour or activity likely to bring the profession into disrepute or undermine public confidence in the profession, may amount to misconduct.

3. The committee will hear the new inquiry into the case of Jonathan Maldwyn Smith (registration number 75431) who on 13 February 2002 at Southampton magistrates’ court following a guilty plea, was convicted of driving a motor vehicle, namely a motor car, on a road after consuming so much alcohol that the proportion of it in his breath, namely 95µg of alcohol in 100ml of breath, exceeded the prescribed limits, contrary to section 5(1)(A) of the Road Traffic Act 1988 and schedule 2 to the Road Traffic Offenders Act 1988.

4. The committee will hear the new inquiry into the case of Joanna Claire Tisdale (registration number 93523) who on 24 April 2006 in Wolverhampton Crown Court was convicted of eight counts of theft.
