Statutory Committee decisions

The Statutory Committee of the Royal Pharmaceutical Society of Great Britain met on Tuesday 25, Wednesday 26 and Thursday 27 September 2007 and made the following decisions:

1. Following the inquiry into the case of Fatehali Abdalhussein Hassanali (registration number 67479) the committee resolved to issue Mr Hassanali with a reprimand. The committee found proved that the supply of 210ml of methadone 1mg/1ml sugar free mixture against a prescription calling for 120ml; a failure to take adequate steps to confirm the identity of the patient before making the above supply; recording a supply of 120ml in (i) the Controlled Drugs Register for Methadone supplied, (ii) the patient’s instalment prescription and (iii) the patient’s medication record, when Mr Hassanali knew that he had supplied 210ml; and following the above erroneous supply, a failure to (i) take any or adequate steps to enquire about the patient’s health and/orwell-being; (ii) take any or adequate steps to recover the excess supply given to the patient; (iii) inform pharmacy staff or his employer of his conduct as set out above, amounted to misconduct.

2. Following the inquiry into the case of Shiv Kumar Sharma (registration number 90720) the committee adjourned to resume at a later date.

3. Following the inquiry into the case of Jonathan Maldwyn Smith (registration number 75431) the committee directed that no further action should be taken.

4. Following the inquiry into the case of Joanna Claire Tisdale (registration number 93523) the committee directed the removal of Ms Tisdale’s name from the Register. The committee found proved a conviction in Wolverhampton Crown Court on 24 April 2006 for eight counts of theft. Under Section 11 of the Pharmacy Act 1954, the direction to remove Ms Tisdale’s name from the Register is not to take effect until the expiration of a period of three months from the date on which notice of removal was given or in a case where an appeal has been brought against the direction, until the appeal is determined or withdrawn.
