Disciplinary Committee

The Disciplinary Committee will meet at the Royal Pharmaceutical Society of Great Britain, 1 Lambeth High Street, London SE1, on Monday 21, Tuesday 22, Wednesday 23, and Thursday 24 January 2008 at 9.30am to hear the following Statutory Committee inquiries:

1. The new inquiry into a complaint by the Council of the Society against Matthew Duncan Mollen (registration number 1071281) which alleges that regularly ordering for and/or supplying to an individual Valoid (cyclizine) tablets contrary to the requirements of Key Responsibilities 1 and 2 and Paragraph 10(e) of Part 3 of the Code of Ethics, may amount to misconduct.

2. The new inquiry into Yousef Mohammad (registration number 1066704) who on 31 July 2006 at the Crown Court at Winchester pleaded guilty to and was convicted upon indictment of theft, for which offence he was ordered on 1 September 2006 to (i) pay a fine of £600 within three months, (ii) pay compensation in the sum of £200 to the Alliance Pharmacy within three months, and (iii) contribute £250 towards the costs of the prosecution.

3. The new inquiry into a complaint by the Council of the Society against Salvatore Ferrigno (Registration number 53060) which alleges that: dispensing to a patient four boxes each containing seven buprenorphine (Subutex) tablets 2mg and each labelled as seven buprenorphine tablets 2mg in response to a prescription calling for 50 buprenorphine tablets 200µg; dispensing 28 rosiglitazone maleate tablets 8mg labelled as rosiglitazone maleate tablets 4mg in response to a prescription calling for 28 rosiglitazone maleate tablets 4mg and endorsing the prescription to show that 28 rosiglitazone maleate tablets 4mg had been supplied; dispensing a box containing 50 dihydrocodeine tablets 30mg labelled as containing 100 dihydrocodeine tablets 30mg in response to a prescription calling for 50 dihydrocodeine tablets 30mg; dispensing 94ml methadone mixture 1mg/1ml to a patient in response to an instalment prescription calling for 100ml methadone mixture 1mg/1ml; and, supplying 75ml methadone mixture 1mg/ml in response to a prescription calling for 25ml methadone mixture 1mg/ml, may amount to misconduct.

4. The new inquiry into a complaint by the Council of the Society against Angus McGregor (registration number 62111) which alleges that: the supply of Nuseals aspirin tablets 300mg against a prescription calling for 56 Nuseals aspirin tablets 75mg; the supply of 12 diazepam 5mg tablets and 184ml methadone mixture 1mg/1ml to a patient A which were intended for a patient B, against patient A’s prescription calling for 12 diazepam tablets 5mg; a failure to record in the Controlled Drugs Register the supply of methadone above; a failure to record in the Controlled Drugs Register a later supply to patient B of 184ml methadone mixture 1mg/1ml; the supply of doxazosin 2mg tablets incorrectly labelled as doxazosin 4mg tablets; the supply of atenolol tablets 100mg against a prescription calling for 56 allopurinol tablets 100mg; and, the supply of Isotard 60 XL tablets 60mg, incorrectly labelled as aspirin dispersible 75mg tablets, against a prescription calling for 56 Isotard 40 XL tablets40 mg, may amount to misconduct.
