MEDICAL DEVICE ALERT: BD Luer slip syringes (10 September 2007)

BD Medical Surgical Systems has recently received multiple reports of its Plastipak hypodermic Luer slip syringes 1ml, 2ml, 5ml and 10ml spontaneously disconnecting from, or failing to maintain a secure connection to, Luer fittings on other devices.

There is insufficient replacement stock currently available for the product to be recalled. Recipients are asked to ensure that these syringes are used with caution and to avoid their use in situations where disconnection would pose a high risk to patients or users.

These syringes should not be left unattended while connected to Luer fittings and the manufacturer’s instructions for use (provided on the packaging carton) should be followed to reduce the risk of disconnection.
