Comprehensive websites dealing exclusively with the topic of medicines management are few, but one of the best can be found at www.managingmedicines.com.

This page was set up by Douglas Simpson, former editor of the PJ, and has been maintained since his leaving in 2000. It has an extensive section of resources, links and articles on medicines management as well as regularly updated news about the various pilots schemes around the UK.

Medicines Management (Pharmaceutical care)

Over the past few years pharmaceutical care has emerged as a major topic in pharmacy. The concept of pharmaceutical care was developed in the United States by Douglas Hepler and Linda Strand. It was defined as “the responsible provision of drug therapy for the purpose of achieving definite outcomes that improve a patient’s quality of life” (Am J Hosp Pharm 1990;47:533-43). This was subsequently refined by Strand and co-workers to “a practice in which the practitioner takes responsibility for a patient’s drug related needs and holds him or herself accountable for meeting these needs”. In order to achieve this, the practitioner assesses the reasonableness and effectiveness of the medicines a patient is taking, develops a care plan for the patient and follows up on a systematic basis. The Pharmaceutical Journal has followed and contributed to the growing debate concerning pharmaceutical care within the profession.
